Renal Function in Type 2 Diabetes Patients in Ile-Ife, Southwest Nigeria: A Clinicopathological Study | Chapter 15 | Highlights on Medicine and Medical Science Vol. 18
The goal of this study was to find out how common diabetic nephropathy is in type 2 diabetics in Southwest Nigeria, as well as the clinicopathological correlations.
Descriptive, cross-sectional study design.
Renal Unit, Medicine Department, Obafemi Awolowo University Teaching Hospital, Ile-Ife, Southwest Nigeria, throughout the duration of the study.
Methodology: A percutaneous ultrasound-guided renal biopsy was performed on 40 consenting type 2 diabetic adults out of 88 patients with chronic kidney disease in stages 3 and 4 according to the Kidney Disease Improving Global Outcomes categorization.
Eighty-eight (48.1%) of the one hundred and eighty-three participants had clinical Diabetic Nephropathy, with a mean age of 58.05(6.10) years. Diabetes had been present for 9.55(4.1) years. Protein excretion was 0.55 g/day on average. Glomerular Filtration Rate was estimated to be 45.1(12) ml/min on average. Thirty-nine renal tissues (97.5%) had thickened glomerular basement membranes.The severity of glomerular lesions (r=0.480, p=0.002), interstitial alterations (p=0.602, r0.001), and vascular lesions all had a positive connection with systolic blood pressure (r=0.480, p=0.002) (0.429, 0.006). The glomerular lesions (r=0.500, p=0.001) and interstitial alterations (0.479, 0.002) were associated to diastolic blood pressure. The severity of glomerular lesions (r=0.457, p=0.003), vascular alterations (r=0.483, p=0.002), and diffuse glomerulosclerosis (r=0.407, p=0.009) were all linked to urinary protein excretion. Diabetic retinopathy was associated with glomerular lesion severity (r=0.406, p=0.009) and diffuse glomerulosclerosis (r=0.547, p0.001).
Conclusion: Diabetic nephropathy is very common among type 2 diabetics in Ile-Ife. The most common kidney pathology was glomerular basement membrane thickening. Blood pressure, protein excretion, retinopathy, and renal histology all had a favourable connection. Once diabetes mellitus has been diagnosed, treatment should be multidisciplinary.
